Description of Organization:

The Sunnybrook Veterans Centre has a long history of providing excellent residential care for Canada’s War Veterans. While we continue to provide our Veterans with residential care, we have expanded our care to include short term medically complex transitional care on two units in Kilgour Wing. This transitional care is for patients who no longer require acute hospital care however their care needs cannot be currently met in other community settings.

Job description:

Short Term Medically Complex Care provided within the Veterans Centre is delivered within Complex Continuing Care (CCC) bed type and the anticipated length of stay is dependent on each patient’s complex care needs requiring the expertise of the Veterans Centre interprofessional team. This could be several weeks to a few months.

Our team focuses on helping each patient move towards their goals of care within a functional maintenance model towards a discharge destination. Therefore, discharge planning will start at admission and be part of every team and family meeting. One or two physicians will be the Most Responsible Physician (MRP) for between 18-28 beds (depending on the unit).

Bed assignments by the department chief in consultation with the program chief will reflect alignment with the complexity of the patients’ needs with the training/experience of the hospitalist. Hospitalist training and experience is preferred for the patients with the highest levels of morbidity/treatment complexity.

Physicians will be scheduled on a rotational model (rotations of not less than 2 weeks).

Expectations of MRP:

  • To be on-site daily Monday to Friday including being on-site mid-day at least three days a week in order to attend bedside rounds with the team and be available for both team and family meetings - At other times the physician will be available easily by phone or pager between 0800-1700h
  • Provide an MD schedule for the team
  • Ensure transfer of accountability as per the CPSO policy with MD rotations
  • CPSO Transitions-in-Care
  • Participate equitably in covering on call after hours, weekend and holidays
