• Step 1: Fill out the planning form
• Step 2: Book a meeting with the Patient Engagement Specialist.
• Step 3: Discuss the completed planning form with the Patient Engagement Specialist to determine the best support strategy.
• Step 4: Track patient partner involvement.
• Step 5: Complete the feedback survey.
