UPLEARN: University of Toronto Practice- and Community-Based Learning and Research Network

PBLRN (Practice-Based Learning and Research Network): PBRLNs are groups of learning communities with diverse disciplinary perspectives and expertise that focus on improving delivery and quality of care. PBRLNs are linked to, and deeply embedded in, the communities they serve. As a result, they recognize and are strongly motivated to address inequities informed by a deep understanding of the social determinants of health, and the need to integrate biological, social, and psychological interventions.

EMR (Electronic Medical Record): EMR data is the information that your family doctor enters into their computer system when you go for an appointment. Some data that can be entered in EMR data can be information about a patient’s age, health conditions, medications, vaccines, and allergies.

LHS (Learning Health System): A Learning Health System is when internal data and experience are systematically integrated with external evidence, and that knowledge is put into practice.

POPLAR (Primary Care Ontario Practice-Based Learning and Research Network): A “network of networks” from six university Departments of Family Medicine and the Alliance for Healthier Communities in Ontario.

What does de-identified mean?

De-identified patient data is information taken from a medical record that has had all the direct identifiers removed. Direct identifiers include name, full date of birth, address, and email address. This means that the data no longer contains information that can be used to identify the individual patient.
