Description of organization
The City of Kawartha Lakes (FHT) is composed of a physician FHO of 18 physicians working with a team of allied health professionals composed of FP nurses, NPs, social workers, 2 pharmacists, a dietician, an RT and a foot care nurse.
The FHT also runs a number of useful programs for our patients including Mindfulness, a CHF clinic, a Memory Clinic (assessing for dementia), an Anticoagulation Clinic (for point-of-care INR testing), a Foot Clinic, Exercise and Walking Clinics, a Smoking Cessation Clinic, a COPD/asthma/Healthy Lung Clinic among others.
My practice consists of 3 physicians in one office with an office staff of a receptionist, billing clerk and 2 nurses, one of whom is a Family Practice nurse from the FHT seconded to our office. We cover each other for holidays. Link: https://cklfamilyhealthteam.com
Job description:
Comprehensive family medicine in an office practice Mondays, Tuesday, Thursdays and Friday mornings. Wednesdays are reserved for Nursing Home visits (22 patients).
If desired, this posting could come with an opportunity for coaching on collaborative leadership and team-based primary care from CACHE (Centre for Advancing Collaborative Healthcare & Education).
Hourly/half-day rate: $160/hr; $475/half day
