Enhanced Skills Program: Hospital Medicine Program—Sub-Acute Hospital Medicine Rotation Goals and Objectives

By the end of the rotation the resident will:

Independently assess and admit patients to the sub-acute inpatient service, recognizing and planning for common admission issues, providing follow-up inpatient care and preparing for discharge
Demonstrate an approach to common general medical conditions and their management specifically in a sub-acute hospital setting
Demonstrate an approach to acute medical conditions that present in sub-acute hospital settings including accurate diagnosis and implementing a treatment plan for acute medical issues
Demonstrate an approach to acute medical conditions in the context of the current sub-acute hospital setting and lack of rapid diagnostic testing

By the end of the rotation the resident will:

Incorporate patients into the decision-making process
Deliver understandable information to patients and their families about their clinical course, in collaboration with other team members
Exhibit effective written communication skills (daily notes, admission and discharge summaries)

By the end of the rotation the resident will:

Identify and implement strategies for safe handovers and transfer of accountability for ongoing patient care issues during periods of absence
Actively collaborate with other health care professionals, consultants and medical staff to coordinate the patient’s care
Demonstrate an understanding and awareness of various team roles through collaborative treatment, shadowing and case conferences.
Play an active role in collaborative team meetings, identifying patient needs and resources to meet those needs, including community-based resources

By the end of the rotation the resident will:

Manage time to reflect and balance priorities for ethical and effective patient care
Set realistic priorities and use time effectively in order to optimize professional performance
Make cost-effective use of healthcare resources based on sound judgement

By the end of the rotation the resident will:

Advocate effectively for the biopsychosocial needs of patients and their families within the health care system and community

By the end of the rotation the resident will:

Maintain and enhance professional activities through ongoing learning. Critically evaluate medical information and its sources, and apply this appropriately to practice domains

By the end of the rotation the resident will:

Demonstrate integrity, honesty, compassion and respect for diversity
Demonstrate a commitment to patients and their families, as well as to their profession and to society through ethical practice.
