Fantastic and rare opportunity to join an established FHO associated with a FHT! You will work on a team of 14 physicians in a collegial and supportive office environment. The practice is well-managed with updated CPPs.
Job description:
Practice Size: approximately 1000 patients
Location: West end of Pickering (about 30-40 mins from downtown Toronto; 10-20 mins from Scarborough)
Office Space: Large, clean, fully equipped clinic with 30+ offices and exam rooms, large staff lunch room, multiple staff washrooms, and separate staff entrance
EMR: PS Suite + Pomelo (secure messaging/online booking)
Teaching Opportunity: Associated with the University of Toronto and Queen’s University as an academic teaching site for family practice residents
Parking: Free and easily accessible in shopping plaza for staff and patients
After-Hour Requirements: For FHO-enrolled patients only; shifts split evenly amongst the 14 physicians, usually 1 evening a month from 5-8pm, one weekend (Sat + Fri) every 14 weeks

FHT interdisciplinary staff available to support your patients and practice:
Clinic assistants help room patients, help with vitals/weights/BP
Nursing: Clinic support (injections, preventative care), programs (details below)
Occupational Therapy: Cognitive testing, complex patient support
Hospital Discharge Coordinator: Follow-up of recently discharged patients to offer immediate primary care and update EMR with discharge plan/medications
Pharmacist: Extended drug coverage applications, medication titration support, medication review
Nurse Practitioner: Management of their own patients (rostered to existing FHO physicians) with availability for same-day/urgent appointments for all clinic patients
Select FHT Programs available to support your patients and practice:
Treatment Room: Available daily (Mon-Fri) run by our nursing staff; patients can self-book for injections, TB skin tests, suture removal, other minor procedures
Hypertension: Dedicated nursing staff to provide comprehensive follow-up including lifestyle counselling for patients with hypertension
Smoking Cessation: Dedicated staff to assist your patients in smoking cessation based on the Ottawa model
