Practising Well Community of Practice - an intentional conversation for shared learning hosted by the Ontario College of Family Physicians and the DFCM Division of Mental Health and Addiction.

The Community of Practice was built by family physicians, for family physicians, to connect in real time and learn from others who are working with patients to address their mental health, substance use disorders and chronic pain. These are challenging conditions and treating them has only grown more demanding.
