Description of Organization & Job:
Working with an inter-disciplinary team of nurses, social workers, dieticians, physiotherapist and medical receptionists, the Carefirst FHT delivers exceptional comprehensive care to our communities. We have a robust, Chronic Disease Management and Prevention program, including a Diabetes Education Program, and are actively involved in many of the Ontario Health Team’s primary care initiatives. Our partnerships across the community ensures our patients receive coordinated and high quality of care. Some on-site examples include: Home and Community Support Care Services, MINT Memory Clinic and Ontario Structured Psychotherapy. As a community FHT we collaborate closely with the Carefirst Seniors and Community Services Association and also have unique access to a team of on site specialists including psychiatry, cardiology, endocrinology, optometry, geriatrics medicine, gynaecology, chiropody and audiology.

The Carefirst FHT currently has two locations. Our clinic at 300 Silver Star Blvd is located within a state-of-the-art, newly built “Carefirst One-Stop Multi-Services Centre”. Our other clinic is located at 420 Highway 7 in Richmond Hill and is a well-established and respected practise in this community since 2007. There is an existing wait list of patients so a wonderful opportunity to build up a practise rapidly. New and international graduates (with return of service requirement) are welcome!
Carefirst FHT is also planning expansion opportunities into other communities in Thornhill and Markham. One expansion opportunity will focus on providing comprehensive primary care to individuals with intellectual and developmental disabilities (IDD), physical disabilities and neurodiverse needs.

Required qualifications:
- Family Physician, licensed to practise in Ontario and in good standing
- Excellent communication and interpersonal skills, time management and work ethic
- Ability to work effectively in a health care team
- Compassionate and approachable
- Fluency in second language relevant to the community served (large population of recent immigrants) is an asset
