The Woodbridge Medical Centre Family Health Team (WMC FHT) is a collaborative primary health care team composed of a Dietitian, Family Physicians, Family Practice Nurses, Nurse Practitioner, Pharmacist, Psychotherapists, Social Workers and other Professionals who work together as an interdisciplinary team, to provide patient-led primary health care services and programs.

FHO position Available in FHT with Patient Roster
Full time Associate wanted to provide comprehensive primary care in a Family Health Organization (FHO) payment model.
Roster Size: ~1150 patients
Start date: Available October - but Flexible start date
Well managed practice. Option to grow practice, if desired

7 weeks vacation
