The Woodbridge Medical Centre Family Health Team (WMC FHT) is a collaborative primary health care team composed of a Dietitian, Family Physicians, Family Practice Nurses, Nurse Practitioner, Pharmacist, Psychotherapists, Social Workers and other Professionals who work together as an interdisciplinary team, to provide patient-led primary health care services and programs.
FHO Position Available: 3 month locum opportunity with opportunity to take over practice.
Size: 1150 patients

Fully Staffed: Medical Reception, Nursing, Clinical Aids, Administrative Services Manager, Director of Human Resources & Clinical Operations, Executive Director & Operations Manager and Billing Agent
Full access to mental health and counseling services, social work, Nurse Practitioner, Registered Nurses, diabetes education program, dietician, pharmacist etc.

Schedule
Monday 9 am - 4pm
Tuesday OFF
Wednesday 9 am - 4pm
Thursday 4-9 pm
Friday 9- 5pm
Plus one weekend shift per month either: Saturday 9-3 or Sunday 9-1pm (weekend shifts are walk in).
Compensation:
$1000 per diem + 70% third party billing
$500 per 3-hour block
