Job description:
Summary:
- Three collegial full-time physicians who are part of an Alternate Funding Plan (AFP), seeking a fourth full time physician to provide comprehensive home visits in mid/east Markham and Stouffville. Part time (3-4d/week) may be considered as well.
- Build a roster of 84-96 homebound patients from our waitlist; complete 1100-1300 in-person/virtual appointments per year to meet AFP requirements.
- ~$230,000+/yr + shadow billing and bonuses, minimal overhead (~5% total).
- Team consists of team coordinator, nurse navigators, and part time allied healthcare professionals (pharmacist, OT, chiropodist, community paramedics, home care coordinator liaison).
- Our program utilizes Accuro EMR.
Family Physician Role and Responsibilities:
- Provide virtual and in-home clinical assessments for homebound seniors as their primary care practitioner.
- Oversee medical and clinical care plans for patients, including but not limited to:
- Comprehensive Geriatric Assessment at intake
- Chronic disease management and preventative primary care
- Acute episodic care (in collaboration with team nurses who triage concerns, Advance Practice Nurse who may provide visits, and Community Paramedics)
- Advance Care Planning & Palliative care
- Provide team support by:
- Conducting and documenting Community Paramedic telephone consultations (who conduct routine and urgent visits and point of care investigations on behalf of MRP)
- Attending biweekly interdisciplinary meetings to review patients with acute and/or complex needs
- Providing 3 hours of urgent after hours care weekly as part of AFP requirements
- Attending quarterly meetings for program operations, planning, and evaluation
- Completing monthly administrative documentation for AFP purposes
- Providing mutual vacation coverage for fellow physicians
- Engaging in the training of medical students and residents as required.
Required qualifications:
- Experience in geriatric care and home visits required. COE designation valued but not required.
- Not eligible if part of another rostering model due to the AFP model.
- CPSO, CMPA and liability insurance required, MSH community provider access suggested.
