Interprofessional Primary Care Team (IPCT) Supports

Ontario’s Primary Care Action Plan aims to connect every person in Ontario to primary care by 2029, in large part by building new Interprofessional Primary Care Teams (IPCTs) across the province. The Department of Family and Community Medicine has developed two connected resources to support that work: a provincial Community of Practice and a set of practical guides. Both draw on research our team has led into what's actually working in newly funded IPCTs, and both are built for the people leading and staffing these teams — executive directors, managers, clinician leaders, and frontline staff

Join the IPCT Community of Practice 

The IPCT Community of Practice brings primary care teams from across Ontario together to learn from each other's experience with team-based care. Most sessions open with a short evidence summary, then a panel of leaders and frontline staff from teams who've done strong work in that area — followed by discussion and tools you can bring back to your own team.

Use Our Practical Guides

Linked to our IPCT Community of Practice, each guide tackles a specific topic related to building and running an IPCT. They offer a summary of lessons learned from high-performing Ontario IPCTs, real-world case examples and practical resources that teams can adapt to their context.

The guides provide evidence-informed options grounded in research and implementation science that teams can adapt to fit their unique community, workforce, and stage of development.
