General Description Hospitalist
The Sinai Health Division of Hospital Medicine is seeking locum part-time hospitalists to join the team. Hospitalist(s) will work predominately as Most Responsible Physician (MRP) on Sinai Health’s Reactivation Care Centre (RCC). The RCC is a 16-bed high intensity rehab unit located at West Park’s Buttonwood site. Team members include a nurse practitioner, nurses, therapists, pharmacists, and social workers. On-site requirements for rounding are at least once a week and is flexible. RCC Hospitalists also have the opportunity to rotate onto inpatient units at Hennick Bridgepoint Hospital, a 474-bed post-acute care facility including rehabilitation as well as complex continuing care units. Compensation will be on a fee-for-service model. The positions are available with a start date of January 27, 2025.
Duties and Responsibilities
Patient care activities will include, but are not be limited to:
- Assessment, evaluation, diagnostic determination, on-going treatment and clinical management, necessary referral for consultation, care planning and modification, medications/therapeutic orders, transfer authorization, risk assessment, and, when appropriate, determination of readiness and planning for discharge for inpatients.
- Direct contact and consultation, after patient’s consent, with patient family members, friends, advocates, and others to ensure effective communication of patient status and care plans, and to encourage/support an open visitor- friendly environment within the medical/hospital environment,
- Communication with the patient’s community circle of care as needed to ensure a safe inpatient stay and proper transition back into the community.
- Weekly patient contact/rounding and availability for care management that actively encourages interprofessional care amongst patient care teams is encouraged.
- Accurate and timely completion of all clinical and professional records and documentation to assure compliance with hospital policies, College of Physician and Surgeons of Ontario (CPSO) standards, and OHIP billing requirements.
- Work to create a positive work culture within cross-departmental teams and align this work with other Hospital Medicine initiatives such as quality improvement.
- Teach and supervise trainees in an academic setting
- Perform other related duties incidental to the work described herein.
- Participate in Hospital On-call duties (home call overnight and rounding coverage on weekends).
Required Qualifications at this Level
- Education: CCFP or FRCP certification. Additional training in Hospital Medicine is desirable.
- Experience: Clinical, teaching, quality improvement and/or research experience in the community or academic teaching hospitals is desirable.
- Degrees, Licensure, and/or Certification: Physician will have an active licence in good standing with the CPSO in accordance with the Medical Staff Bylaws of Sinai Health.

- Clinical training and skills as mandated by the CCFP and FRCPC.

- Ability to work with others in a cooperative, interprofessional and collaborative manner.
- Strong written and oral communication skills
- For clinician educators and clinician researchers, strong clinical teaching and research skills respectively
