East End Community Health Centre (East End CHC) provides primary care, health promotion, and disease prevention services for families, children, youth, adults and seniors of diverse backgrounds in the south-eastern quadrant of central Toronto.
We reach out to people in our community who may have difficulty accessing health care due to factors such as language, culture, poverty, mental illness, or the lack of health insurance. We focus on the whole person in our approach and recognize that factors such as income, education, relationships and housing play a role in physical and mental health.

Job description:
The Primary Care Physician provides comprehensive, client-centered, primary health care services as a member of the interdisciplinary team to individuals and families who may experience barriers to good health and wellness.
The Primary Care Physician will function within their scope of practice in accordance to the standards of the College of Physicians and Surgeons of Ontario (CPSO), and to guidelines and policies established at East End CHC. The Primary Care Physician typically assists clients with long-term management of chronic health care conditions, including mental health issues.
Required qualifications:
- Medical degree and family medicine residency from a recognized university
- Licensed to practice medicine in Ontario
- Must be currently registered and in good standing with the College of Physicians and Surgeons of Ontario
- Membership and Certification with the Canadian College of Family Practitioners (CCFP) is preferred
- Current liability insurance through Canadian Medical Protective Association (CMPA)
- Demonstrated ability, commitment to and knowledge of community health care and health promotion
- Experience in primary care settings (community-based preferred)
- Ability to deal effectively with people experiencing crisis and/or mental health challenges
- Able to work independently, and to work effectively in an interdisciplinary team of clinical and program staff
- A strong team player with excellent communication, time management, problem-solving, and conflict resolution skills
- Excellent interpersonal skills with clients in a culturally diverse practice population
- Demonstrated knowledge of and sensitivity to the impact of social, economic, environmental, and cultural issues on health
- Proficiency in the use of computers and experience in using EMR (preferably PSS)
- Second language, relevant to catchment area populations, is an asset – particularly Spanish
- Vaccines (COVID-19 and others) are a requirement of the job unless you have an exemption on a medical ground pursuant to the Ontario Human Rights Code
