Don Mills Family Health Team (FHT) is a well-established, multidisciplinary primary care practice located in the heart of North York at Don Mills and Eglinton. Our team takes pride in fostering a collaborative and supportive work environment, where physicians work alongside Nurse Practitioners, Nurses, Social Workers, a Dietitian, an Occupational Therapist, a GP Psychotherapist, a Patient Navigator, and dedicated administrative staff. This locum position offers a full-scope practice with a rostered patient panel of approximately 1,000 patients, spanning all ages.
Coverage: 5–6 half-days per week (3 hours per half-day), including one after-hours clinic typically held on Monday evenings.
EMR: Accuro with Ocean referral integration.
