Our department is strongly committed to the care of the inner city population.

In addition to providing care and teaching in all aspects of basic family medicine, our staff has developed expertise in a number of areas in response to the needs of the community we serve. These areas include:

  • HIV primary care
  • Addiction medicine
  • Lesbian/gay/bisexual/transgendered (LGBT) health
  • Care of people who are homeless or under-housed
  • The poor as well as adolescents
  • New immigrants

Clinical activities in these areas are fully integrated into the day-to-day practice of family medicine. We are now working in an FHT model in close collaboration with professionals from nursing, social work, psychology, chiropractic, dietitians, pharmacist and nurse practitioners. All our sites use the Practice Solutions EMR software.

St Michael’s Hospital provides compassionate care to all who enter its doors. The hospital also provides outstanding medical education to future health care professionals in 27 academic disciplines. Recognized areas of expertise include the following:

Wellesley - St. Jamestown Health Centre at 95 Homewood Avenue

Sumac Creek Health Centre at 73 Regent Park Blvd

  • DFCM’s newest clinic opened in July 2015
  • Diverse and growing patient population, including the priority neighbourhoods of Regent Park, Moss Park and St. Jamestown
  • Interprofessional team-based approach to care, including many FHT-wide services
  • Strong orientation to social determinants of health in day-to-day care
  • Co-located with SMH lab, diagnostic imaging and FOCUS mental health services

St. Michael’s Hospital is a tertiary care teaching hospital. The hospital shares its neighbourhood with both the wealthy financial and cultural districts of Toronto and some of the poorest areas of the city. It offers a wide range of speciality and subspecialty services both in the inpatient and ambulatory settings.
