Interprofessional Primary Care Team Community of Practice

Interprofessional Care Teams (IPCTs) bring together a range of health professionals—including family physicians, nurse practitioners, nurses, social workers, pharmacists and other allied health providers—to deliver coordinated, team-based primary care that better meets the needs of patients and communities.

The Community of Practice is delivered through a partnership between Ontario Health and the Primary Care Collaborative, with support from the Department of Family and Community Medicine. Together, we provide coordinated leadership, aligned priorities and collective oversight for the initiative.
