New Staff Award of Excellence

DESCRIPTION

This award is for an early career staff member who has joined the Department of Family and Community Medicine or Teaching site at the University of Toronto within the last three years and has exhibited strong performance and significant potential for career development.

ELIGIBILITY

Any staff member whose primary role is to support the academic mission of the DFCM, and who works in the DFCM central office or at a fully affiliated or community affiliated teaching site is eligible. 

Please note, for the purposes of this award, staff are defined as providing administrative support in advancing the teaching, research and educational programs of DFCM. Clinical staff (such as HPEs) are not eligible for staff awards. Administrative staff whose role is to provide support for clinical programs are also ineligible for these awards. If you require clarification regarding eligibility, please contact DFCM.awards@utoronto.ca before proceeding with the nomination process.

Eligibility for this award requires completion of the employee's probationary period and application before the three-year mark of employment.

SELECTION CRITERIA

The selection committee will consider the following criteria:

  • Demonstrates excellence through the support of department administration and/or teaching and research endeavours
  • Work is held in high regard by colleagues, staff, learners, faculty and senior administration.
  • Evidence of continued contributions to the success of the Department and/or outstanding contributions to the success of a single event, project or initiative of importance to the DFCM.
  • Demonstrates capacity for leadership, collaboration and/or quality improvement and innovation>

SELECTION CATEGORIES

One award winner will be selected from each of the following categories:

  • Central Department

  • Fully Affiliated Sites

  • Community Affiliated Sites

NOMINATION

Nominations can be made by any faculty or staff member of the DFCM, including self-nomination. The nominations will be considered by the DFCM Central Awards Committee. The nominator is responsible for the collection and submission of the award nomination package and informing the nominee of their nomination.

Nominations for this DFCM Award of Excellence must include the following:

  1. Completed nomination form that includes the supporting signature of a teaching site Chief, DFCM Program Director, Division Head or Supervisor.

  2. Two letters of support indicating why this individual is being nominated in the selected category. Letters that are the most helpful address the criteria for the award and present evidence supporting the nominee’s excellence in those criteria.

  3. CV.

  4. dditional material documenting evidence of excellence may be submitted. A maximum of 3 additional documents can be added to the nomination. These may include letters of support, project proposals, educational materials, etc.
